At Lone Star Family Health Center, our mission is simple — to provide high-quality, compassionate and affordable healthcare to individuals and families across our community. As a trusted nonprofit health organization, we believe everyone deserves access to excellent healthcare, regardless of income, background or circumstance.
Founded in 2002, LSFHC offers a full range of medical, dental and behavioral health services for patients of all ages. Our team of dedicated providers works together to keep our patients healthy at every stage of life — from preventive care and routine checkups to chronic disease management and specialty services.
We are proud to serve our communities through five active clinic locations in Conroe, Willis, Spring, Grangerland and Huntsville. And in early 2026, we’ll expand our reach even further with a new location in Magnolia, continuing our commitment to make quality healthcare accessible to more families across the region.

Beyond the exam room, LSFHC is proud to serve as a teaching health center, training the next generation of family physicians through our nationally recognized Family Medicine Residency Program. By investing in education, innovation and community outreach, we’re helping to build a stronger, healthier future for Texas families.

Our History
The roots of Lone Star Family Health Center stretch back to the 1970s, when Conroe Family Medicine Residency Program was established to train physicians in community-based, outpatient care. The program emphasized hands-on learning, compassionate service, and a deep connection to the local community — values that continue to define our organization today.
As the residency program grew, so did the need for a dedicated clinical home to support both patient care and physician training. In 2002, that vision became reality with the formation of LSFHC, a nonprofit organization created to provide high-quality, affordable healthcare while continuing to educate the next generation of family physicians.


Under the leadership of Dr. Alvin Jones, LSFHC’s first chief executive officer, the organization achieved a major milestone in 2004 by earning designation as a Federally Qualified Health Center. This achievement expanded access to care for individuals and families throughout Montgomery County and beyond — ensuring everyone, regardless of their financial situation, could receive comprehensive medical, dental and behavioral health services.
Today, LSFHC continues to honor its dual mission of training exceptional family physicians and delivering compassionate, patient-centered care. From our beginnings as a teaching clinic to our role as a cornerstone of community health, we remain committed to serving with excellence, empathy, and integrity.
