Insurance Information

At Lone Star Family Health Center, we believe everyone deserves access to excellent healthcare, regardless of income, background or circumstance.

At LSFHC, we accept most private insurance, Medicaid, CHIP and Medicare as well as cash payments. We offer a sliding fee discount program based on eligibility. Contact our financial assistants before your appointment to see if you qualify.

Uninsured? No problem. Our Sliding Fee Discount Program allows us to “slide” or reduce fees for eligible patients. We encourage you to set up an appointment with our eligibility team to assess whether you qualify for free or discounted services.

Eligibility for the Sliding Fee Discount Program is determined by a patient’s household size and annual income. An application must be completed, and proof of income is required. Discounts are applied based on federal poverty income guidelines, updated annually by the federal government. While fees for eligible patients are reduced, a nominal fee for services will be the patient’s responsibility. Payment is expected at the time of service.

While our goal is to provide affordable care for all our patients, payment must be made at the time of service. Not having payment at the time of service may result in your appointment being rescheduled.

Health Insurance Plans Accepted

If you’re unsure whether your plan is accepted, our team is happy to help — just give us a call at 936.539.4004 or ask at your next visit.

Please remember to show us your insurance card when you arrive for your appointment. If you do not let us know about your coverage, you will be charged for past visits. For patients with a plan that needs a primary care provider (PCP) to be listed, either LSFHC or a LSFHC provider must be your PCP with your plan.
