New Patient Resources

Lone Star Family Health Center would like to welcome you as a new patient to our health center! We provide high quality services to individuals and families with Medicaid, CHIP, Medicare, private insurance and those who are uninsured. To get started, we require all new patients to follow our patient registration process.

New Patient Registration Process:

Insured? Call us at (936) 539-4004 to register and schedule your first appointment today. You also can reach a member of our patient registration team via e-mail for questions about insurance, health care providers, appointment times and more.

Uninsured? No problem. If you are new to our health center and uninsured, we need you to set up an appointment to speak with our eligibility team to assess whether you qualify for free or discounted services. Eligibility for the Sliding Fee Discount Program is determined by a patient’s household size and annual income.

Patients with insurance also may be eligible for discounts and are encouraged to apply.

Sliding Fee Application

Before Your First Appointment

Approximately 48 hours prior to your appointment, you will receive a text message asking you to confirm or cancel your appointment. We understand that life is unpredictable, so appointment cancellations are needed from time to time. We ask that when you need to cancel an appointment, you let us know by responding to our text message or giving us a call at (936) 539-4004.  

Approximately 24 hours prior to your appointment, you will receive a text message and/or e-mail message with an option to check-in and complete any required paperwork ahead of your appointment. If you are unable to complete paperwork electronically before your appointment, we ask that you arrive 15 minutes early to get this done in the clinic before you see the doctor. 

What if I have an urgent need for a same-day appointment?

We offer same-day appointments for established patients of the health center and do our best to see patients on a walk-in basis when possible. Please note, our doctors are scheduled to see patients by appointment all day long so if you arrive to a clinic requesting to be seen as a walk-in, you may be waiting for an extended period of time or asked to schedule an appointment for a future date. Our Conroe clinic location offers walk-in hours from 5 pm to 9 pm each weekday and 9 am to 1 pm on Saturdays.
