New Patient Resources

Lone Star Family Health Center would like to welcome you as a new patient to our health center!

Whether you are coming for family medicine, women’s health, behavioral health, or dental, we provide high-quality services to individuals and families with Medicaid, CHIP, Medicare, private insurance and those who are uninsured.


New Patient Registration Process:

To get started, we require all new patients to follow our patient registration process.

  1. Schedule your first appointment by calling us at 936-539-4004.
  2. Uninsured? No problem. Contact our financial assistance team to see if you are eligible for our Sliding Fee Discount Program. Call us at 936-539-4004 to schedule an appointment or email us.

  3. After your appointment is scheduled, you can reach a member of our call center team via email or your patient portal (after portal registration) for questions about insurance, providers, appointment times and more.

Before Your First Appointment

  • Approximately 72 to 48 hours prior to your appointment, you will receive an email and/or text message asking you to confirm or cancel your appointment.
  • Within 24 hours prior to your appointment, you will receive an email and/or text message with an option to check-in and complete any required paperwork ahead of your appointment. If you are unable to complete paperwork before your appointment, we ask you arrive 15 minutes early to fill out any paperwork.

When you visit, please bring with you:

  • One form of ID (Driver’s License, Photo ID, Passport, etc.)
  • Insurance card if applicable
  • Any medications you take, including vitamins
  • Immunization record if applicable

What if I have an urgent need for a same-day appointment?

  • We offer same-day appointments for established patients of the health center and do our best to see patients on a walk-in basis when possible.
  • Please note, our providers are scheduled to see patients by appointment all day long so, if you arrive to the health center requesting to be seen as a walk-in, you may be waiting for an extended period of time or asked to schedule an appointment for a future date.
  • Our Conroe location offers walk-in hours from 5 pm to 9 pm each weekday and 9 am to 1 pm on Saturdays.
