If you need an official copy of your medical records for yourself, another provider, your insurance or another purpose, please complete a Medical Records Request Form below.

Please complete all required fields on the form so we can:
- Confirm your identity
- Understand which records you need
- Know where and how to send your records
Be sure to sign and date the form. Unsigned forms cannot be processed.
Step 3: Send Your Completed Form
You can mail or fax your completed form to our Medical Records Department.
