Pharmacy

We have a full service pharmacy with many specialty services available exclusively to patients of the Lone Star Family Health Center.

Here is a sample of what we offer our patients:

  • Conveniently located inside the clinic for one stop convenience
  • Private counseling room
  • Registered pharmacists on duty daily
  • Pharmacy Assistance Programs
  • Two full time on-site eligibility counselors who assist Lone Star patients in managing prescription costs
  • On site consultation between Lone Star pharmacists and your Lone Star physician to optimize your medications
  • Specialty pharmacy services
  • Anticoagulation clinic for monitoring patients on Warfarin (Coumadin) and other anti clotting medicines
  • Medication consultations and medication therapy management services
  • The Lone star Pharmacy accepts
    • Most private insurance
    • Medicaid; and Medicare part D
    • In house sliding fee prescription card for eligible patients

Anticoagulation Clinic

We have an on-site anticoagulation (Coumadin) clinic:

  • Operated by a Licensed Pharmacist
  • Education on drug and diet
  • On site lab with results at time of visit
  • Referrals accepted

Pharmacy Assistance Programs

We have 2 full time eligibility counselors who will guide you through the process of obtaining medications through drug manufacturers. They will sit down with you and go over necessary qualifications and available medications. The eligibility counselors do all the work by contacting your doctor for a prescription and signing the application.

Medication Therapy Management (MTM)

Medication Therapy ManagementWe work with several insurance companies to provide one on one counseling with a licensed pharmacist concerning the patient’s medication profile. The patient’s insurance company contacts our pharmacy to let us know which patients qualify for this service. The pharmacist will then contact the patient’s doctor to discuss their medications. The pharmacist will contact the patient and discuss a time that this service can be provided to the patient. This service is completely free to the patient and ensures proper education concerning medication usage.
