We want to be the medical home for you and your entire family. We can provide all aspects of your medical care from delivering babies and caring for children to treating adults.

Insurance

We take most plans.

Medicare

We take Medicare and most Medicare supplements and HMO plans.

Medicaid/Medicaid HMO

We take most Medicaid and Medicaid HMO plans as well as CHIP and CHIP Perinatal.

Health Care Assistance Program (HCAP)

We accept HCAP through Montgomery County Hospital District. Your co-pay is due at check-in.

Self Pay

We provide discounted fees for payment prior to service for self pay patients.

SPECIAL PROGRAMS AND DISCOUNTED SERVICES

Sliding fee scale, discounted prenatal care, family planning and breast & cervical cancer screening:

Our sliding fee scale covers doctor visits, x-rays in the clinic, and prescriptions filled in our pharmacy at discounted fees based on household income. Our grant funding can help cover the cost of screening and treatment for cervical & breast cancer, vasectomy and other birth control. Lab tests must be paid for in advance by sliding fee and self pay patients. Other patients may receive a bill from an outside lab company. Co-pays are due at the time of the visit. Financial counselors are available during business hours to apply for these discounted programs.

Although we offer programs to help with the cost of care, we are not a free clinic. Fees are due at check-in.
