Diabetes Education

We are focused on a team approach at LSFHC, which provides the best, most lasting results for patients. Our specially trained staff works with the patient, family and physician to personalize each care plan. With education as the foundation, proper diabetes management is an attainable goal.

Diabetes education focuses on 7 self care behaviors that are essential for improved health status and greater quality of life.

Insurance

Medicare/Medicaid and most private insurance companies provide coverage for diabetes self management training (DSMT).

Appointments are available from 8:30 am to 3:30 pm, Monday through Friday.
