It is the mission of Lone Star Family Health Center to provide high quality, easily accessible health services to the underserved and the community at large.
A Brief Overview of Lone Star Family Health Center
LSFHC was formed January 28, 2002 to pursue Federally Qualified Health Center (FQHC) status in order to have a stable organization with a mission to strengthen, stabilize and expand patient access to healthcare services in Montgomery County. LSFHC became fully operational as an FQHC on February 28, 2004. In 2007, LSFHC had 35,182 patients visits in the clinic and another 11,230 hospital visits. LSFHC also serves as a nationally recognized family medicine residency program that graduates seven residents per year. Of the graduates, more than half stay in Montgomery County to practice.

Lone Star Family Health Center Service Area and Eligibility
LSFHC primarily serves residents within Montgomery County. LSFHC’s target population represents the 26 most economically depressed and underserved census tracts in the county. LSFHC takes most insurance plans, Medicare, Medicaid, and the county Health Care Assistance Program. Uninsured patients are eligible for a sliding fee discount if they are below 200% of the federal poverty level.
Community Outreach and Collaboration
At LSFHC we feel that care does not end at the clinic’s walls. LSFHC has a dedicated Outreach Director that participates in health partnerships such as Heart of Montgomery County and Gateway to Care and serves on numerous community committees such as the Homeless Coalition, Hispanic Achievement Advisory Committee and Disaster Recovery Task Force. The staff at LSFHC consistently participates in numerous health fairs throughout the year as well as Shots Across Texas, The Harvest Outreach Festival and many other events throughout Montgomery County

Directions to Lone Star Family Health Center
Lone Star Family Health Center is located off of FM 2854 next to the Conroe Police Department.
If going north on I45, take the Gladstell exit. Stay on feeder road and go over the railroad tracks. At the light, turn right on 2854 and the clinic is about a 1/4 mile down on the left.
If going south on I45, take Hwy 105 exit and stay on the far right feeder road. Take it to FM 2854. Take a left on 2854 and go under I45. The clinic is about 1/4 mile down on the left.
