Community Outreach

Being an active member of the Montgomery County community is a big part of what we do at Lone Star Family Health Center. We work on specific projects that help make getting health care easier and also attend various community events throughout the year.

Community Outreach Programs

logo-susan-g-komenProject Pink is an effort Lone Star Family Health Center (LSFHC) has undertaken as a funded partner of Susan G. Komen® Houston. Through this partnership, LSFHC provides free mammograms to uninsured women twice each month at its Conroe location and at various outreach events throughout Montgomery County.

The goal of Project Pink is to provide breast health education, screenings and diagnostic services to uninsured, low-income women in and around Montgomery County. The project is designed to increase the number of women who detect breast cancer early, when survival rates are higher.

Free mammograms are offered to qualifying women between the ages of 46 and 64 through partnerships with and The Rose. Any uninsured woman may call LSFHC and request a free mammogram. Women must speak to our Case Manager to assure that they meet both financial and medical eligibility requirements.

Project Next Step is the next step in health for new moms. Regular preventative check-ups for both babies and moms are vitally important, especially during the child’s first year.  Establishing a medical home for a newborn protects their health by ensuring they receive all necessary vaccines, monitor their development to allow for early detection and treatment of any problems, and helps address the health disparities experienced by low-income and minority children.  But medical care for new moms is just as important.  New moms remain at-risk for medical conditions and mental health issues that threaten their health during that first year as well.  Establishing a medical home for new moms can help to ensure that they remain physically and mentally healthy and are up to the important task ahead – raising a healthy family.

Project Next Step comes in the form of an insulated bag filled with gifts for both mom and baby. This bag will be delivered to each new mom by her doctor before she leaves the hospital. Doctors will remind each mom of the importance of both her and her baby receiving ongoing medical care, and also let them know about the ways Lone Star Family Health Center (LSFHC) can assist with the financial burdens of healthcare.

Project Next Step is funded by the LSFHC staff, a generous donation by LSFHC faculty, and a grant from the Montgomery County Community Foundation.
